I. Information for citizens of the European Union (EU) and / or European Free Trade Association (EFTA) Member States:
In order to benefit from free healthcare in Poland, a Student - Citizen of the above-mentioned countries must have:
1. The right to free treatment in Poland is confirmed by the EHIC card (polish EKUZ-europejska karta ubezpieczenia zdrowotnego) - i.e. the European Health Insurance Card.
2. Foreigners, EU and EFTA citizens should obtain a card in their country of origin before coming to study in Poland.
II. Information for citizens of countries other than European Union (EU) and European Free Trade Association (EFTA) countries:
In order to benefit from free healthcare in Poland, a Student - Citizen of the above-mentioned countries must have:
1. Health insurance policy covering the costs of treatment during education in Poland
or
2. A voluntary health insurance contract concluded with the National Health Fund (NFZ).
In order to sign a voluntary health insurance contract, you should go to the NFZ branch competent for the place of residence.
The branch competent for foreigners living in Szczecin is the Branch of the National Health Fund with its seat at ul. Arkońska 45, 71-470 Szczecin.
(Important !!! - before visiting the National Health Fund, you should - in the Student Affairs Department of the Academy of Art in Szczecin, download a certificate of student status to be submitted to the Department of the National Health Fund.
The certificate is issued within 7 days from the date of oral notification or submission of a written form for a certificate).
3. Students who have concluded a voluntary health insurance contract with the National Health Fund are obliged to go to the Social Insurance Institution (ZUS), where they will complete the ZUS ZZA declaration - Application for health insurance, no later than 7 days from the date of signing the contract. After submitting the declaration, the student receives a form ZUS DRA - Settlement Declaration and information about the individual account number for settlements with ZUS at the Social Insurance Institution.
In order to apply for health insurance, go to the Social Insurance Institution (ZUS) Branch:
1. ZUS at ul. Matejki 22, 70-530 Szczecin
or
2. ZUS at ul. Andre Citroena 2, 70-769 Szczecin (Prawobrzeże).
ATTENTION !!!
After performing the above-mentioned activities and receiving the above-mentioned s (Agreement on voluntary health insurance with the National Health Fund, ZUS DRA declaration, information about the account for settlements with ZUS), together with the above-mentioned s, you should go to the Employee Affairs Department of the Academy of Art in Szczecin (2nd floor, room 203) so that the University could transfer the amount of the student's health insurance contribution to ZUS every month.
